چکیده
1 of 2 DESCRIPTION A 57-year-old marathon runner presented with orthostatic dizziness and headache, which appeared just after running a few kilometres and had persisted for 7 days. No other neurological abnormalities were found. On lumbar puncture, the initial pressure was zero. A cranial MRI with gadolinium showed a marked enhancement of the dura mater ( fi gure 1 , arrows). MR cisternography and holocord MR myelography revealed a cerebrospinal fl uid (CSF) leak at the right C3 and C4 spinal root sleeves ( fi gure 2 , arrows). The patient was confi ned to complete bed rest under the administration of necessary intravenous fl uids for 14 days. On the 14th hospital day, orthostatic symptoms had resolved and MR myelography demonstrated the disappearance of the CSF leak ( fi gure 3 ). He resumed occasional running 6 months later in the absence of any recurrence of the symptoms and a follow-up cranial MRI with gadolinium confi rmed the disappearance of dura mater enhancement ( fi gure 4 ). CSF leak syndrome is usually caused by various degrees of trauma. 1 2 The strain of running had not been excessive for this individual, but the mode of onset suggested that running must have had some infl uence on the initial appearance of this disorder. Although blood patching of the dura mater is focused on as a method of treatment for this condition, 2 3 in some cases recovery is seen after a certain number of days of complete bed rest. 2 If accurate Images in...

Endoscopic transpterygoidal repair of a large cranial defect with cerebrospinal fluid leak in a patient with extensive osteoradionecrosis of the skull base: case report and technical note.
BACKGROUND Endoscopic endonasal techniques have recently become the method of choice in dealing with cerebrospinal fluid leak involving the anterior cranial fossa. However, most surgeons prefer an intracranial approach when leaks involve the middle cranial fossa. This case report illustrates the possibilities of using endoscopic techniques for cerebrospinal fluid leaks involving the middle foss...
متن کاملA Prospective Study To Evaluate The Role Of Perioperatve Cerebrospinal Fluid Diversion By Lumbar Drain In Transnasal Transsphenoidal Tumor Surgeries
Background &Aim: The endoscopic endonasal transsphenoidal approach has been widely employed as a treatment for pituitary adenomas and other parasellar tumors. However, the major concerns related to endoscopic endonasal transsphenoidal surgery include the risks of CSF leak post-operatively leading to morbidity and at times mortality due to severe meningitis. Several studies in the past have foun...
متن کاملSpontaneous cerebrospinal fluid leak of the sphenoid sinus mimicking allergic rhinitis, and managed successfully by a ventriculoperitoneal shunt: a case report
BACKGROUND Spontaneous cerebrospinal fluid leaks are rare but may lead to confusion with other diseases in patients without history of trauma. We report a rare case unusual for two reasons. First, our patient was put under antiallergic medication for months before the diagnosis of spontaneous cerebrospinal fluid leak of the sphenoid sinus. Second, our patient was managed successfully by a ventr...
متن کاملManagement of intra-operative cerebrospinal fluid leak following endoscopic trans-sphenoidal pituitary surgery.
OBJECTIVE Cerebrospinal fluid leakage is the most common complication of endoscopic trans-sphenoidal pituitary surgery. However, there is no uniformly accepted way of managing this complication when it occurs intra-operatively. This paper describes a quick, simple technique, involving layered fibrin glue and gelatin sponge, which does not compromise post-operative patient follow up. METHOD Re...
متن کاملCerebral gray matter volume deficits in first episode psychosis.
BACKGROUND Structural brain differences including decreased gray matter and increased cerebrospinal fluid volumes have been observed in the brains of chronically ill patients with schizophrenia. We hypothesized that deficits in gray matter volume would be present in patients presenting with a first episode of nonaffective psychosis. METHODS We used magnetic resonance imaging to compare the br...
